Patients are typically contacted for scheduling within two business days of referral. Referrals are not required to be seen at our office or scheduled for procedures, however some insurance companies require a referral - please check with your insurance company if needed. Please call with any questions or specific requests.

Dr. Zach Hartman is in-network with many health insurance plans including Anthem BlueCross/BlueShield, Cigna, Aetna, and United. Our practice also accepts Medicare, Medicaid, TriCare, and self-pay.
